how is this achieved. What program, and how do they add real diamond to it, cause i have seen some with baggettes in them very detailed.

Program: Photoshop.

I doubt it very much that it's real diamond, mapped or whatever. Sidefx had a logo with that exact effect, maybe he can help you.
