Don't Know how to do this Text Effect (Really Cool)

Can someone replicate (as close as possible) this text effect?

I just realized I forgot the top shadow, but you should be able to handle that. If not, let us know and we can help out. If I don't make it back here right away, someone else could take the PSD and modify it.
