Tab effect. How to do?

Well. I've seen this forum and found it the very best of the world about Photoshop. Congratulations. My question is the following:

I wanna make a tab effect. But what I got so far does not match what I wanna(need to) do. Look a image of example:

I wanted to EXACLY the way this image is. How to do it?

PS: In the section "Ask Mr. PS" I couldn't make him this question. Why? The forum tells me that I have no permission todo it...

I dont see an example, but Robouk has a nice tutorial on how to do that msn kind of tab.
