how do you get a stroke to start think and end thinner?

Yeah, use the transform, like distort or perspective(?) and then adjust. It may not come out real clean, but your can tweak it a bit. You would have better results using Illustrator if you have it, then you could open it up in PS.

Brush Options...Fade and Set number of steps...will require some experimentation.
