Okay, Here we go

Okay, Im really confused from my previous post, so I am just going to give you my code and ask that someone look at it and tell me what I need to correct. For some reason, the buttons on the left wont stay together and break up when I type the text in the middle. Ive tried v align and it doesnt seem to work. Maybe Im putting it in the wrong place but could someone help me out please so I can stop bothering everyone, lol .
