Form Script

Okay I don't have a clue on how to start this so I'll just start. I'm an ESL teacher and I'm looking for a new contract. Now what I would like to do is have a form on my website that the director of the school could fill out with many different fields in it. Now the thing is that when they say select their country I want the page to refresh and retain what has already been filed in, so that they have the list of cities, provinces, states pertaining to them. Now not only do I wan it to do that I want it to send an auto response and i want it to send what they have filled in to me but I want it sorted by it's content, so that only certain ones will go to a certain email address.

Does any of the this make sense??

So what I'm looking for is a script to do this or something very similar. Please help!

i don't know any script that does this off the top of my head, sorry.

Either I suppose. I have PHP on my server so I could use that. Why do you have a suggestion for me??
