Website Contact Menu

Hey everyone, I'm really having a difficult time finding a tutorial on how to make a contact menu. I’d like the person who looking through my site to be able to put there email address in, type a comment and have it email me.

So far I've done searches on google and I've been looking through the dreamweaver exchange at macromedia for quite awhile and I haven't had any luck.

I've seen this type of thing on some of your guy's websites. Know where I can learn how to do this for my site?

Help would be appriciated. Thanks guys. :D
